ICD-10-GM code as given by the referring physician (Verordnung/Überweisung). This tool does not diagnose — confirm the exact code and any 4th/5th-digit qualifier against the prescription and current ICD-10-GM catalogue.
Document the measured finding alongside the code: ROM in degrees, MRC strength, pain score, gait distance or relevant functional test. The qualifier must reflect clinical assessment.
